Manual trans as well as auto trans,the fluid should be changed once a year and/or every 30k miles.

You have a bad transmission control module (TCM). It is under the passenger side front floor mat. You can get rebuilt units off Ebay, whatever you do, don't get a used one! They are all bad.

Remove the plug and drain it. If it doesn't have a plug, remove the pan. Clean the screens and replace the gaskets with new ones. Refill.
