SC or IM DHE, which can be self administrated by patients is more effective than intranasal DHE for severe migraine attacks but produces more side effects ( Boureau et al., 2000; Colman et al., 2005; Touchon et al., 1996 ). IV DHE is often used in combination with an antiemetic drugs such as IV metoclopramide for intractable migraine or status migrainosus (a debilitating migraine attack for >72 h) For the clinical entity of migraine, DHE can be used in patients who do not respond to or tolerate triptans or who are withdrawing from triptans or analgesics because of overuse ( Boureau et al., 2000; Colman et al., 2005; Touchon et al., 1996 ). DHE appears to be effective in reversing central sensitization, which could explain why it does not appear to induce medication-overuse headache ( Boureau et al., 2000; Colman et al., 2005; Touchon et al., 1996 ). However, clinical trials have shown that SC or nasal spray DHE is inferior to SC or nasal spray sumatriptan ( Boureau et al., 2000; Colman et al., 2005; Touchon et al., 1996 ). Of note, DHE should not be used within 24 h of ergot-like agents or triptans. Similar to ergotamine, DHE is contraindicated in patients with uncontrolled hypertension, ischemic heart disease, and in pregnancy or breastfeeding.

Oral DHE is poorly and erratically absorbed, and consequently of limited value in acute treatment of migraine. As a nasal spray, the bioavailability of DHE is about 32% of that obtained with an injection. DHE nasal spray is marketed as a 4-mg dose, with 2 mg being sprayed into each nostril. Clinical trials of DHE nasal spray demonstrated 2-hour pain-relief rates ranging between 61% and 30% vs placebo rates between 23% and 33%. 120 Associated symptoms of nausea, photophobia, and phonophobia were reduced relative to placebo, and there were lower recurrence rates of migraine relative to triptans. Nasal DHE is generally well tolerated, with the most common adverse events being rhinitis, dizziness, altered taste, site reactions, nausea, and vomiting. Often an anti-emetic such as metoclopramide is combined with DHE to minimize nausea, especially when administered intravenously.

In a single comparator study, SC sumatriptan was slightly more efficacious and provided earlier headache relief than DHE, but DHE was associated with less recurrence of migraine. 119 Given the potential heterogeneity of the migraine population, mechanisms exploring these unique subsets could prove to be a line of further investigation.
