To be eligible you must: • Have Medicare Part A and Part B (by age or disability). You must continue to pay your Medicare Part B premium (unless paid for under Medicaid or by another third party). • Reside in the service area (the state of Minnesota). • Not have end-stage renal disease (kidney failure), in most cases.
