The topic of corneal edema is important for clinicians to understand because it affects the architecture and function of the entire cornea. 1,5,6 Epithelial edema clinically causes a hazy microcystic appearance to occur in the epithelium in mild-to-moderate cases of corneal edema ( Figure 9.1A ), significantly decreasing vision, and increasing glare. It can also cause the development of large painful, subepithelial bullae in severe cases of corneal edema ( Figure 9.1B ). Stromal edema clinically appears as a painless, cloudy, thickening of the corneal stroma ( Figure 9.1B ), resulting in a mild-to-moderate reduction in visual acuity and an increase in glare. At the same time, Descemet's membrane folds commonly appear on the posterior surface of the cornea, particularly in severe cases of corneal edema ( Figure 9.1B ).

OVERVIEW
The back layer of the cornea is made up of endothelial cells which keep the cornea clear. All cataract surgery (even “perfect” surgery) does some damage to these endothelial cells. Most corneas have plenty of “extra” endothelial cells, so a small degree of endothelial cell loss from cataract surgery doesn’t usually cause any problem.
DEFINITION
Corneal swelling (edema) that develops after cataract surgery and doesn’t resolve over several months.
SYMPTOMS
Blurred vision, may worse in the mornings, which may clear over a few hours, or may be present all day long. It often improves over the 1 st few months after cataract surgery, but it may not. The swelling can cause painful blisters as the condition progresses.
CAUSES
Damage to the cells on the back layer of the cornea (endothelial cells) related to the cataract surgery. Even “perfect” cataract surgery can occasionally cause corneal swelling that doesn't resolve on its own.
RISK FACTORS
History of Fuchs dystrophy prior to the cataract surgery is by far the most common risk factor for corneal swelling after cataract surgery. Other eye surgeries such as glaucoma surgery or retinal surgery, or prior eye trauma, also increase the risk of corneal swelling after cataract surgery.
TESTS AND DIAGNOSIS
The diagnosis can usually be made during a slit lamp examination. Ancillary testing can include measurement of the corneal thickness (pachymetry) and endothelial cell imaging (specular microscopy).
TREATMENT
Treatment ranges from observation in mild cases to salt drops and ointment (hypertonic sodium chloride 5%) to surgery including partial thickness corneal transplant (e.g.
Practical advice for both routine and complex cases
Providing care for your patients during their recovery from cataract surgery can be exciting and gratifying. Few experiences will cement patients to your practice like regaining their vision; it will also help your clinic operate at the peak of its capacity. Most patients have a straightforward recovery, and only a few require more attention.
