What is Hemi inattention?
Hemiinattention is defined as the failure to report or respond to stimulation located contralateral to a brain lesion when the failure cannot be attributed to a primary sensory loss or motor impairment (Heilman et al. 1985).

Hemispatial neglect results most commonly from strokes and brain unilateral injury to the right cerebral hemisphere, with rates in the critical stage of up to 80% causing visual neglect of the left-hand side of space.

1B: Ask Month and Age
Ask the patient what the current month is and what their current age is. The answers must be correct. There is no partial credit for being close.
2: Horizontal Extraocular Movements (Best Gaze)
Hold your finger about a foot in front of the patient's face and ask them to follow it. Slowly move it up and down and side to side. Check their eyes for abnormal activity.
3: Visual Fields
Hold up a certain amount of fingers about a foot in front of, but slightly to the side, of the patient's head. Ask them to tell you how many fingers you are holding up. Do this in four separate corners creating a virtual square around their head.
4: Facial Palsy
Ask, or use pantomime (express with movement) to encourage, the patient to show their teeth or raise their eyebrows and close their eyes.
7: Limb Ataxia (loss of full control of bodily movements)
Hold your finger about two feet in front of the patient's face. Ask them to touch the tip of your finger with their own finger and then touch their nose. Repeat this with the other hand.
8: Sensation
Using a safety pin, gently poke each extremity and both sides of the face. After each poke, ask if they felt anything.
10: Dysarthria (slurred or slow speech that is difficult to understand)
If patient is thought to be normal, an adequate sample of speech must be obtained by asking them to read or repeat words from a list ( Listed here ).
What does it mean when a patient feels a pinprick?
1 = Mild-to-moderate sensory loss; patient feels pinprick is less sharp or is dull on the affected side; or there is a loss of superficial pain with pinprick, but patient is aware of being touched. 2 = Severe or total sensory loss; patient is not aware of being touched in the face, arm, and leg.
What does 1 mean in a patient's slur?
1 = Mild-to-moderate dysarthria; patient slurs at least some words and, at worst, can be understood with some difficulty.
What is personal hemineglect?
Personal hemineglect involves recognition of only one half of the body, with neglect of the other side. For example, the patient will groom, comb, and respond to visual stimuli on the ipsilesional side of the body only.
How is homonymous hemianopia different from visual neglect?
Homonymous hemianopia tends to sharply obey the vertical meridian, whereas visual neglect represents a gradient of inattention that may cross over into the ipsilateral hemispace. Furthermore, patients with homonymous hemianopia are usually aware of the visual field defect and will often exhibit compensatory mechanisms such as head-turning and eye movement to compensate for the visual defect. By contrast, patients with visual neglect are unaware of the neglected visual field and do not attempt to compensate for it. However, these distinctions may be blurred in cases where homonymous hemianopia and visual neglect are both present, requiring more detailed testing to identify the presence of both syndromes.
