The inguinal canal is a passage in the lower anterior abdominal wall located just above the inguinal ligament. It starts from the internal inguinal orifice, extends medially and inferiorly through the abdominal wall layers and ends in the external inguinal orifice.

The inguinal ligament is in the pelvis. It’s actually two symmetrical bands. Each one starts at the anterior superior iliac spine (toward the back of the wing-like bones of the pelvis). The ligaments then run in and down at an angle to the pubic tubercle — that’s at the bottom center of the pelvis, very deep in the groin.

Clinical significance

Abdominal contents (potentially including intestine) can be abnormally displaced from the abdominal cavity. Where these contents exit through the inguinal canal, having passed through the deep inguinal ring, the condition is known as an indirect or oblique inguinal hernia. This can also cause infertility. This condition is far more common in males than in females, owing to the inguinal canal's small size in females.
