Recognition of a perianal, rather than a buttock, abscess may have prompted earlier exploration of the local anatomy, which ultimately expedited the diagnosis in this case. Although inspection of the perianal area was felt to be normal, no rectal examination was performed.

Outcome

The patient underwent an elective colostomy at 10 months, followed by excision of the presacral mass several weeks later ( figure 2 ). Intraoperatively, a fistula was identified which extended from the presacral mass to the perianal region, which had been the source of the recurrent abscesses. Colostomy closure was at 13 months.
