The usual duration of Lovenox treatment duration therapy is 8 days or until hospital discharge. 1 mg/kg/day subcutaneously, in patients <75 years of age with severe renal impairment. Refer to the PI for patients ≥ 75 years of age. Lovenox is not intended for intramuscular administration.
